Areivim USA Claims Policies and Procedures

  1. The following sets forth the general policies and procedures followed by Areivim USA upon the unfortunate passing of an Areivim USA member. These policies and procedures may be changed or amended from time to time, and at any time, at the sole discretion of Areivim USA.

  2. Upon becoming informed of a member’s death, Areivim USA will reach out to the member’s family to begin the process of confirming eligibility.

  3. Areivim USA will send a consent form and a questionnaire to the contact person on record. The consent form will request that a single individual be appointed as the representative of the member’s family for all interactions with Areivim USA. The questionnaire will request various details regarding the Niftar/Nifteres including, among other things, information concerning the cause of death, onset of illness, medical history/records and other family information.

  4. It is critical that all requested medical information and records be provided in order for Areivim USA to be able to confirm eligibility for collection of benefits. As noted in the Terms and Conditions of Areivim USA’s membership, Areivim USA will request medical records from prior to the time of enrollment upon a member's passing to determine whether a fund should be collected since collections will not be undertaken on behalf of a member who had a life-threatening illness at the time of enrollment for membership.

  5. The medical information and records provided to Areivim USA are reviewed by medical professionals. Areivim USA will determine eligibility for collection of benefits on a case-by-case basis. If the representative indicates that no medical information is available or is unable to provide such information, a meeting with the Rabbinical Board is required before the claim process can proceed.

  6. If approved for collection, Arevim USA will arrange for a collection of funds to be distributed in accordance with the Terms and Conditions of Areivim USA’s membership.

  7. If a claim is denied, the representative of the member will have the opportunity to, if so desired, meet with the Rabbinical Board to contest such determination provided the representative has provided the medical information requested by Areivim USA, which will be provided to the Rabbinical Board for purposes of such meeting. At such meeting, the representative will have the opportunity to present any medical information or records the representative may wish to proffer to counter the Rabbinical Board’s determination. The representative is required to execute a binding arbitration agreement/form prepared for the meeting with the Rabbinical Board. The representative will not be provided any confidential medical reports or summary prepared by any medical professional on behalf of Areivim USA or the Rabbinical Board.

  8. Not less than three members of the Rabbinical Board shall be present at such meeting. The representative of the member will have no right to request any specific member of the Rabbinical Board be present at the meeting. The Rabbinical Board may only either approve or deny the claim in its entirety.

  9. Following the meeting described in the preceding paragraphs and after duly considering any new information provided by the representative, Areivim USA will communicate to the representative the final determination of the Rabbinical Board. Such final determination shall be binding on all parties.
